All Stars Commitment Certificates are now being created by using Microsoft Word template files. Links to these files are available below. Two versions of the commitment certificate templates are available: 1) family life included and 2) family life excluded. Please download the template that corresponds with the version of All Stars you are currently using.

Within the downloaded files will be a document with explicit directions on how to produce the commitment certificates for your All Stars students. The file is called Commitment_Forms_Directions.doc.
